Privacy Policy — Public DOT Scheduling

Hamilton Mobile Chiropractic uses this DOT Physicals website to provide business information and receive scheduling requests. This notice explains the limited information handled through the public scheduling form.

Information you choose to provide

The scheduling form collects your first and last name, at least one contact method (phone or email), and any optional scheduling note you enter. The system also assigns a request identifier and submission time so that it can confirm receipt and avoid duplicate entries.

Do not include medical information. Do not submit diagnoses, medications, medical history, driver-license information, insurance information, medical records or other sensitive health information in the public scheduling form. It is not a patient portal or clinical intake form.

How we use the information

We use scheduling-request information to respond, coordinate an appointment, maintain request records, prevent duplicate submissions, protect the service from abuse and troubleshoot delivery problems.

Storage and service providers

Scheduling requests are sent through our website server to a business-controlled Google Workspace workflow and stored in a Google Sheet used by authorized practice personnel. Providers supporting website hosting, Google Workspace, communications, security and scheduling may process information while providing those services.

Internal staff notifications contain an opaque reference to a new request, not the contact details or scheduling message entered in the form.

Responding to your request

We may use the phone number or email address you provide to respond about your scheduling request. This public form is not presented as consent to receive unrelated marketing messages.

Technical information

Our hosting and security systems may process technical information such as IP address, browser or device information, timestamps, requested URLs and security logs. We use this information for service delivery, rate limiting, reliability, fraud prevention and troubleshooting.

The public landing page may request font files from Google. Google may receive ordinary technical request information under its own privacy practices. If you choose an external link, the destination service applies its own privacy practices.

Cookies and browser storage

Restricted staff and intake areas may use functional session or security cookies. Limited interface preferences may use browser storage. The public scheduling form does not use browser storage to retain your contact details or Notes.

When information may be shared

We may disclose scheduling-request information to service providers acting for us, when you direct us to do so, or when reasonably necessary to comply with law, protect rights and safety, or investigate misuse.

Retention

Retention depends on operational, security, recordkeeping and legal needs. Clinical records, if later created through a separate secure process, are governed by separate notices and retention requirements. Contact us with a question about a scheduling request.

Your choices

You may schedule without using the web form by calling (619) 323-5020 or emailing info@hamiltonchiro.net. You may also contact us to request access, correction or deletion of scheduling-request information, subject to applicable legal, security and recordkeeping requirements.

Security

We use administrative and technical safeguards intended to protect scheduling requests. No internet transmission or storage system can be guaranteed completely secure.

Third-party links and later clinical processes

The website may link to services operated by other organizations, which have their own privacy practices. If you proceed to a separate secure clinical intake or become a patient, additional privacy notices and consent documents may apply. This page describes the public scheduling form; it is not a HIPAA Notice of Privacy Practices for clinical records.

Changes and contact

We may update this notice as the website or our practices change. The effective date above identifies the current version.
